1. Acceptance of Terms

By booking an appointment, receiving treatment, or otherwise engaging with All Body Health & Wellness (“Clinic,” “we,” “us,” or “our”), you (“Client,” “you,” or “your”) agree to be bound by the following Terms and Conditions, our Privacy Policy, and any other applicable policies or consents provided.

2. Services Provided

We offer health and wellness services with a focus on medically supervised weight management using GLP-1 receptor agonists such as Semaglutide and Tirzepatide (the “Services”). All medications are prescribed based on clinical appropriateness, medical history, and provider discretion.

3. Medical Disclaimer

• GLP-1 treatments require a medical evaluation and are not suitable for everyone.
• Results vary by individual. Weight loss and other health outcomes are not guaranteed.
• You must disclose your full medical history, current medications, allergies, and any contraindications.
• Our services are not a substitute for primary care or emergency medical services.
• We do not provide emergency care. If you are experiencing a medical emergency, call 911 immediately.

4. Eligibility

You must be:

• At least 18 years of age (or have written consent from a legal guardian).
• Medically cleared by one of our providers following a consultation.
• Willing to comply with all follow-up appointments and medical monitoring.

5. Appointments & Cancellations

• Appointments must be scheduled in advance.
• We require 24 hours notice for cancellations or rescheduling. Late cancellations or no-shows may be subject to a fee of $50.
• We reserve the right to reschedule or cancel appointments due to provider availability or unforeseen circumstances.

6. Fees & Payments

• All fees are due at the time of service unless otherwise arranged.
• Prices are subject to change without notice.

7. Refund Policy

Due to the nature of our services and medications, all sales are final. We do not offer refunds on consultations, treatments, packages, or unused medications once dispensed, except where required by law.

8. Telehealth

Some consultations or follow-ups may be conducted via telehealth. You agree to:

• Provide accurate information and attend telehealth sessions in a private, safe environment.
• Have access to a device with internet, audio, and video capabilities.
• Understand the limitations of telehealth and the importance of in-person evaluations when necessary.

9. Risks & Side Effects

GLP-1 medications may cause side effects, including but not limited to:

• Nausea, vomiting, diarrhea, constipation
• Headache, fatigue
• Risk of pancreatitis, thyroid tumors (including medullary thyroid carcinoma), or kidney problems
• Hypoglycemia (especially if used with other diabetic medications)

You will receive full information on risks, benefits, and alternatives during your consultation and consent process.

10. Termination of Services

We reserve the right to terminate the provider-client relationship at our discretion, especially in cases of:

• Noncompliance with treatment plans
• Abusive behavior toward staff
• Failure to disclose accurate health information
• Illegal or unethical conduct

11. Privacy & Confidentiality

We adhere to HIPAA and other applicable privacy laws. Your health information is protected and will only be shared with your consent or as required by law. Please refer to our full [Privacy Policy].

12. Limitation of Liability

To the fullest extent permitted by law, All Body Health & Wellness shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to the use of our services, including but not limited to adverse reactions, missed appointments, or delays in treatment.

13. Governing Law

These Terms are governed by the laws of the State of Arkansas, without regard to conflict of law principles. Any disputes shall be resolved in the courts of Faulkner County, Conway, AR.

14. Changes to Terms

We reserve the right to modify these Terms at any time. Updates will be posted on our website or provided in writing. Continued use of our services after changes constitutes acceptance of the new terms.
